📋 How This Form Works

This form collects the information needed to generate your customer's Completion Pack — a set of official documents required to close out an external wall insulation project. The pack contains 3 documents:

1. ESBN Safety Notice — notifies the homeowner that ESB cables were temporarily relocated and ESB needs to return to re-fix them. Requires contractor and homeowner signature.
2. MEV Safety Notice — SEAI-required notice confirming mechanical extract ventilation was discussed before wall insulation was installed. Requires installer and homeowner signature.
3. FAST ETICS Warranty Card — 10-year product warranty covering all FAST insulation system materials. Distributor signature only — no homeowner signature required.

Once saved, you can email the homeowner a secure signing link or print the pack to collect a signature in person. The homeowner reviews the full PDF before signing — a single signature covers both documents that require one. The homeowner should retain a copy of the ESBN Safety Notice in case their home is selected for an SEAI inspection.

📄 Document 1 — ESBN Safety Notice

Informs the homeowner that ESB Networks temporarily relocated the electricity supply cables to allow external wall insulation to be installed. ESB will return within several weeks to re-fix the cables permanently. If ESB has not returned within 4 weeks the homeowner should call 1800 372 757 quoting their MPRN and ESBN ID.

📄 Document 2 — Mechanical Extract Ventilation Safety Notice

SEAI requires this notice when mechanical extract ventilation in wet rooms (kitchen, bathroom, utility) has not been installed before wall insulation. It confirms the installer discussed MEV options with the homeowner in accordance with Chapter 10 of NSAI SR 54. Both parties must sign. Retain for any future SEAI inspection.

📄 Document 3 — FAST ETICS Warranty Card

A 10-year product warranty issued by P.W. FAST covering all materials in the insulation system (adhesives, base coats, primers, plasters and paints). The distributor signs on behalf of FAST. The homeowner keeps this card as proof of warranty — they do not sign it. Note: after 24 months the homeowner may request an on-site warranty review (cost: 200 PLN, credited back if defects are found).
